The description of charges are as follows:
Summary of Charges - a summary of monthly charges billed for local service, adjustments, other charges and credits, directory advertising, payments, long distance, late fees, taxes, and the total amount due. Balance forward and payments received are printed. Post net bar codes and customer by name and address information is printed.
Service and Equipment – a listing of the items making up monthly local service charge. Printing this page is optional. The customer’s carrier charges are also printed on this page.
Adjustments– a detailed explanation of all miscellaneous adjustments. This is only printed when a customer has current activity.
Long Distance Calls – a listing of all toll calls separated and identified by carrier logo or carrier and sub-carrier names with the following information:

Additionally, Call Plan Summary pages follow the affected carriers toll statements. The required 900 messages are printed when 900 calls are present. Disconnection notices are printed based on the subscribers past due status.
Return with Payment – this page is returned with the subscriber’s payment. A special message area is provided for controlling past due accounts or for any other purpose chosen by your telephone company. If the late pay calculation option is being utilized, a late payment message is printed. If the auto-payment option is chosen, a special message appears advising the customer not to pay this bill. Bar code for payment scanning is printed on this page.
Statements may be sorted by telephone number or zip code. Another feature is the ability to group statements by subscriber type to assist in stuffing.
Statements to PDF
Midwest Data offers your statements in PDF file format for easy viewing and quick response. You can see the exact statement that was sent to your customer. The Adobe PDF file format is an industry standard and easy to use. You can easily search current and past statements for better customer care. The Adobe Acrobat reader is free and universal.
The file can be sent to you in one of four ways, whichever is most convenient for you.
